as a Physical Therapist in home health. You will have the
opportunity to get to know your patients well. This role allows you
to be an integral part of their care team. Average patient visits
are 45 minutes, providing significant time for meaningful therapy.
You will enjoy broad independence while being part of a cohesive

evaluate, re-evaluate, and treat patients. You will coordinate and
communicate treatment activities with interdisciplinary team

accordance with agency policies and professional standards of care.
Effective communication with all members of the healthcare team is
