Service Areas:We have openings for our teams in Manchester and Nashua areas and one for our team in Milford, Hollis, Brookline and surrounding communities. Should you live in these communities, consider working close to home! ResponsibilitiesCompletes an initial home health assessment of patient and family to determine home care needs. Provides a complete physical assessment and history of current and previous illness(es). Utilizes the nursing process in planning, providing and evaluating skilled needs.Completes a comprehensive patient assessment that is reflected in accurate OASIS scoring.Develops a care plan, which establishes goals based on nursing diagnosis and incorporates therapeutic, preventive, and rehabilitative nursing actions. Includes the patient and family on the planning and revising process. QualificationsGraduate of an accredited school of nursing.Associate degree in nursing.Registered as RN with New Hampshire's Board of Nursing.A minimum of two years' recent acute care experience.Ability to exercise initiative and independent judgment. Preferred QualificationsBachelor's degree in nursing.Community Health/Education experience.Certifications: IV, Chemotherapy, Ostomy, as examples.Familiarity with OASIS and what drives reimbursement.EMR software proficiency. RequiredRegistered as a RN with New Hampshire's Board of Nursing.CPR Certification.Satisfactory criminal record.Satisfactory motor vehicle record.
