http://schemas.microsoft.com/office/2004/12/omml" xmlns="http://www.w3.org/TR/REC-html40"> This Nurse Practitioner (NP) is primarily responsible for providing comprehensive and high quality medical care to the complex and diverse older adult who is located in skilled nursing facilities for acute or long term care. In addition, the NP may also see participants in the community and at the centers as needed. The NP serves as a vital member of the PACE Interdisciplinary Team in the coordination and provision of care for medically complex and diverse older adults taking into consideration the individual's unique goals. The NP also participates in the care planning process along with the interdisciplinary team in order to satisfy the requirements set forth by the PACE Model of Care. Essential Responsibilities:Performs skilled nursing facility visits regularly in accordance with Element Care standards and state nursing home requirements.Completes initial, periodic and annual physical examinations of the participant located in long term care.Evaluates the needs of the participants and develops treatment plans in concert with the Interdisciplinary Team.Identifies and documents the participant's goals of care along with end of life wishes as appropriate.May also evaluate participants for routine and acute reasons at the center or at the individual's home as needed.Uses telehealth as needed and when appropriate.Participates in the after hours on-call rotation.Provides appropriate health care and culturally competent teaching to participants, families, caregivers, & the Interdisciplinary Team.Adheres to the standards set forth in the Element Care NP/MD Collaborative Practice Guide.Completes appropriate, accurate, timely documentation.Ensures that duties are accomplished in a fiscally responsible manner adhering to established budgetary constraints.Ensures that all interactions with participants are conducted in a manner that adheres to the highest standards of care for all Element Care clients.Ability to pass an N95 mask fit test. Position requires mask where seal is critical. Candidate is required to not have facial hair that interferes with a tight seal of the respirator.Frequent local travel.Performs other duties as required. Job Specification:Masters of Science in Nursing (MSN).Current Board Certification through ANCC or AANP & licensure as a Nurse Practitioner in MassachusettsCurrent Massachusetts and DEA Prescription Privileges. Suboxone Waiver preferred.Current CPR Certification.Bilingual/bicultural (Spanish & English) required.At least one (1) year of experience working with a geriatric population required. At least one (1) year of experience working with low income and diverse cultural groups preferred.Previous skilled nursing facility experience preferred.Obtains and maintains appropriate contracted facility credentials and privileges.Demonstrated ability to work within urban communities with strong emphasis on racially, ethnically, and culturally diverse, low income, and special needs populations.Ideal candidate is detail oriented, takes initiative, problem solves, and is able to complete assignments on time with minimal direction.Proficient in treating participants with multiple chronic and acute illnesses.Ability to work well within a team concept and to be respectful of coworkers and colleagues.Ability to communicate clearly and succinctly with other team members.Covid vaccines required.
