This Nurse Practitioner (NP) is primarily responsible for providing comprehensive and high quality medical care to the complex and diverse older adult who is located in skilled nursing facilities for acute or long term care. In addition, the NP may also see participants in the community and at the centers as needed. The NP serves as a vital member of the PACE Interdisciplinary Team in the coordination and provision of care for medically complex and diverse older adults taking into consideration the individual's unique goals. The NP also participates in the care planning process along with the interdisciplinary team in order to satisfy the requirements set forth by the PACE Model of Care. Bilingual Spanish preferred .
Nurse Practitioner Qualifications:
Masters of Science in Nursing (MSN).
Current Board Certification through ANCC or AANP & licensure as a Nurse Practitioner in Massachusetts
Current Massachusetts and DEA Prescription Privileges. Suboxone Waiver preferred.
Current CPR Certification.
Bilingual/bicultural (Spanish & English) required.
At least one (1) year of experience working with a geriatric population required.
At least one (1) year of experience working with low income and diverse cultural groups preferred.
Previous skilled nursing facility experience preferred.
Obtains and maintains appropriate contracted facility credentials and privileges.
Demonstrated ability to work within urban communities with strong emphasis on racially, ethnically, and culturally diverse, low income, and special needs populations.
Ideal candidate is detail oriented, takes initiative, problem solves, and is able to complete assignments on time with minimal direction.
Proficient in treating participants with multiple chronic and acute illnesses.

Ability to work well within a team concept and to be respectful of coworkers and colleagues.
Ability to communicate clearly and succinctly with other team members.
